You will need a substitute sticker if the old one is messed up or  Missing or not readable:

To apply for a substitute Sticker, You must bring the following with you:

1. Registration Card Or Tag/Plate# Or Vin#
2. Driver’s License
3. Form VR-009–(Note: This form will be given to you here to sign, & date)

    Substitute Tags/Plates

 Substitute tags can be applied for, if you are missing one of your tags or both or if the tag(S) is not readable or messed up or Stolen.

 Bring the following with you:

1. Old Plate(s) /Tag (s)
2. Driver’s License
3. Form  VR-009– (Note: This form will be given to you here to sign, & date)
